Concordance Home Care closely supervises all our caregivers. For the duration of care, your caregiver will follow the care plan that was designed for you. If your caregiver has a question or believes there should be a change in the care plan, he or she will contact their supervisor immediately. We will evaluate the situation, discuss potential changes with you, and implement any agreed upon alterations.
During the first month of care, we follow up with clients on three separate occasions either by phone or in-home visit. After the first month, we check in once a month to make sure all is going well. Should the need arise, a Concordance Home Care Supervisor is on call 24-hours a day to handle any problems or concerns you may have.
